1.Client Profile & Case Details

HR (Hypnotic Responsiveness) : 8/10
V/A/K Modality : Visual 5, Auditory 9, Kinaesthetic 1 (predominantly Auditory)
Eye Roll Test : 2+1 (medium depth of trance)
Physical Distress (हाथ) : 10/10
Emotional Distress : 10/10
Therapist’s Clinical Impression : Chronic depressive presentation, deep fear state।

Safety

Focus : Rapport, pre-talk, assessment, primary history taking

Focus : Continuation of history taking, assessment, theme finalisation

Focus, जैसा मैंने तय किया था : Induction, relaxation, visualisation, present-life regression work, age regression, affect bridge, और उसके बाद past-life exploration

past-life exploration नहीं हुआ, इसलिए KMF2C, carry over की पहचान, resolution और inner child work, इनमें से कुछ भी इस सेशन में नहीं हो पाया।

Deepening

Healing Work

Emotional Distress :

Res.

Therapist’s Note

A Sleeping Ocean Between Black and White

PLRT Case Report 2

Greetings to all the seniors.

This is my second PLRT case report. I have tried, to the best of my ability, to follow the Amarantos 15-stage protocol. Please read it and tell me whether I was able to do my work.

The entire session was conducted by me in Hindi, since that is the language both the client and I are comfortable in. This report too is originally in Hindi, and here is its English translation.

Client Profile & Case Details

Name : Abhinav
Gender / Age : Male, 52 years
Occupation : Working in a private company
HR (Hypnotic Responsiveness) : 8/10
V/A/K Modality : Visual 5, Auditory 9, Kinaesthetic 1 (predominantly Auditory)
Eye Roll Test : 2+1 (medium depth of trance)
Physical Distress (hand) : 10/10
Emotional Distress : 10/10
Session Theme (in the client’s own words) : Feeling stuck and trapped, and wanting relief from the problem in his hand
Therapist’s Clinical Impression : Chronic depressive presentation, deep fear state. This is my own assessment, not the client’s statement, and not a formal diagnosis.
Total sessions : 3 (on three consecutive days)
Total duration : About 9 hours (2:30 + 3:00 + 3:30)

Risk Assessment and Safety

It was recorded during case history that Abhinav has recurring thoughts of ending his life. He told me this himself, in his own words.

Source : Client’s own statement, during intake

Case History

Abhinav is living with a neurological condition called focal dystonia. The problem began in 1998, when he was appearing for the PSC Mains examination for the first time. From then until now, that is for about 28 years, it has kept growing worse.

His right hand is affected. Earlier the effect was limited to his fingers, now the whole hand is caught in it. The fingers suddenly stiffen. Holding anything is difficult, he cannot grip a pen, and writing is almost impossible. Using a mobile phone, locking a door, picking something up, all of these are hard. When his stress rises, the problem worsens.

Abhinav wants relief from this physical difficulty, and it was with this purpose that he came to the PLRT sessions.

Worth noting : the moment at which the symptom began does not look like coincidence. The hand stops responding in exactly that examination where he had to prove himself, and prove himself by writing.

Life Profile (CL)

Abhinav comes from a middle-class family and holds a mid-level post in the private sector. He began life with the dream of entering the administrative services, but did not succeed. First his parents’ hopes, then the family’s high expectations, and then his own expectations of himself, all three kept him under pressure.

With half his life gone, he today considers himself only partly successful. For his under-performance, not only his family but he himself looks upon himself as an offender.

When the pressure grew heavy, Abhinav began presenting himself as something he is not in reality. In social life a friendly man, a party man, cheerful, sociable, confident. And on the inside an over-thinker, someone who holds grudges, frightened, weighed down by a sense of inferiority, and entirely alone.

The habit of living with two faces became so settled that he has forgotten how to feel and accept himself properly. Caught in this conflict between the display and the reality, he now feels trapped, completely stuck. He is unable to find any way out.

The facts in the description above are what the client gave me. The reading of two faces is mine.

The pattern from Samskara to Vasana, as I saw it :

Repeated humiliation and the sense of failure, from which an impression settled inside, from which came the tendency of never letting pain out, and from that, today’s double face.

Session Structure

Session 1, duration 2:30 hours

Focus : Rapport, pre-talk, assessment, primary history taking

Session 2, duration 3:00 hours

Focus : Continuation of history taking, assessment, theme finalization

Session 3, duration 3:30 hours, the final session

Focus as I had planned it : Induction, relaxation, visualization, present-life regression work, age regression, affect bridge, and past-life exploration thereafter

What actually happened : Induction and deepening, an unsuccessful attempt to elicit an HM, visualization, an attempt at age regression towards 1998, and then guided imagery, healing work, grounding and closure.

What could not happen : the affect bridge did not carry forward. We never reached in-utero or LBL exploration. Past-life exploration did not take place, and so KMF2C, the identification of carry over, resolution and inner child work, none of these could happen in this session.

I am recording this incompleteness here deliberately, because the true account of the session is made of exactly this.

Everything described below is from Session 3.

Induction and Deepening

The final session began with all the protocols and procedures in place. After complete relaxation, when I understood that the client was in that depth of trance from which one can move towards the root question of the problem, I began the sequence of questions, answers and suggestions.

Attempt to elicit HM : intending to begin with a happy experience, I asked whether there was any event that still gives him joy. No answer came from his side. HM 1 could not be obtained.

I then suggested that he imagine a garden and place himself there. From there I began moving in the direction where answers connected to the theme might lie.

What I saw later : the absence of an HM was itself information. That not one accessible happy memory came forward said a great deal, to my mind, about his inner state, even before the case history did.

Session Record

Scene 1, the seashore

T : Are you able to see or hear anything? Any image, any sound, any movement at all?
C : There is a sea there. I am standing at the edge of the sea.

T : Is anyone else with you?
C : No, I am completely alone there.

T : What all are you able to see there?
C : The sea is swaying in front of me. I am standing on the sandy shore. There is a cool breeze, and sunlight is shining overhead.

T : How are you feeling at this moment?
C : It feels good. I feel completely relaxed.

T : Anything else you are able to see there?
C : A banyan tree is visible at some distance. There is no one for as far as I can see. I am completely alone.

T : Does this place have any connection with your life? Have you ever been here before?
C : No, this is a new place for me.

T : Anything else you can see or hear here?

(No answer. Complete silence.)

After this I tried to take him directly to the time when the symptom began.

T : Abhinav, let us go to 1998 now. Remember that time when you were appearing for the Mains examination and the problem in your hand began for the first time. What kind of experience were you going through then? What were you feeling?

(The client stayed silent at this question. He said nothing.

But there was movement in his body language. Behind the closed lids there was a quickness in the eyes. Something looked as though it were being swallowed back at his throat. It seemed to me that something was going on inside, but was not finding words.)

T : That is all right, Abhinav. It is not necessary that every experience be given words. Just stay with this experience of yours for a while. Then we will try to move ahead.

(Scene 1 ends here.)

Scene 2, a village and fields

T : What place is this, where you are seeing yourself right now?
C : It is some village. I do not know what place this is.

T : What are you able to see or hear here?
C : There are some fields, of flowers. Roses have bloomed there, perhaps.

T : How does it feel, having arrived here?
C : I feel good. Sitting on the ridge of the field, I feel a great deal of peace.

T : Is anyone else there apart from you?
C : No one is with me. Some labourers are working in the field.

T : Do you recognise those labourers?
C : No.

T : Is any one of them looking towards you?
C : No, they are all busy with their work.

T : Does this place have any connection with your present life?
C : No. I have never come here before this.

T : Any image, any event, anything here that you dislike or that is troubling you?
C : Everything is fine here. I am happy at this moment.

T : Would you like to say whether you came here for some purpose?
C : No, just like that. I liked this place.

T : Anything else you are able to see here?
C : There is a pond, a large one, brimming with clean water.

T : Is this pond familiar to you?
C : No. But there was a pond just like this in our village. We used to bathe there in childhood.

My decision at this point

We could not move any further. Complete blockage. No SE came forward, no affect or CO appeared to connect, and AA was out of the question altogether.

I was feeling that the client was avoiding the zone of his sorrow and bitter experience. It seemed the session was not arriving anywhere. The emotional ground for catharsis was not forming, nor had we reached any scene that would support it.

In that situation, rather than pulling the CF Pattern forward, it seemed right to take the session towards healing mode. For this I used two metaphors, one the pond and the other an imagined hillock. The pond was material the client had given himself, so I carried that forward.

Scene 2 continued, healing work

T : Abhinav, imagine that right in front of this same pond there is a small hillock, and that steps have been built to go up it. Now we will slowly move towards those steps.
(Count down)
You are at the top of the hillock. Look around a little, what can you see from there?

C : There is a tree near me, and in front the pond looks like a sea.

T : Very good, Abhinav. Now imagine that right beside where you are standing there is a wooden box. This box is filled with your troubles. You will have to lift this heavy box with all your strength, and then release it into the swelling water. Watch the box being released into the water. See how your troubles are dissolving into it.

T : After releasing the box, are you feeling lighter?
C : I cannot say.

T : That is all right, Abhinav. It is not necessary that we feel something every time. Come, let us walk slowly down from the hillock.

T : What can you see?
C : The pond is visible in front.

T : Very good, Abhinav. Just as you used to bathe in the pond in childhood, and used to feel good every time after bathing, let us repeat those happy memories here once again.

(The childhood memory of the pond was used as an anchoring resource. It was the only positive memory that surfaced in the session.)

T : With a count from ten to one, you will go down deeper into the pond.
(Count down)

T : Now you find yourself shoulder-deep in the clean water of the pond. Let yourself relax in this beautiful pool. No obstruction, no tension. Just be with yourself.
Now you will gently dip under the water. You are under the water. With a count from three to one, all your remaining troubles, all obstructions, all impurity will dissolve like clay, and you will come out of the pond completely refreshed.
(Count down)
Leaving all your problems and your low feelings right there, come out of the pond slowly, entirely empty. Outside the pond there is a clean smooth stone. Sit there comfortably and enjoy the mild sunshine. Perhaps something will feel lighter, and perhaps it will not just yet. Either way is fine.

(Formal grounding and closure of the session followed.)

Outcome

Physical Distress (hand) : before the session 10/10, after the session 5/10
Emotional Distress : before the session 09/10, after the session 4/10

Client’s post-session response :

He said he did not feel he was experiencing any past life. The same doubt had been arising in me during the session.

He said one more thing, which turned out to be the most important for me. In his words, whenever I wanted to say something about my problems, my throat would close up and my mind would go numb, as though someone were stopping me from speaking.

Two days later :

I was in touch with the client again after two days. He reported three things.

The frequency of the focal dystonia cramps has come down.

The heaviness in his chest has reduced. (This was not mentioned at intake; it came up for the first time at follow-up.)

The way he talks to himself has become healthier than before.

Of the three, the last one seemed most important to me. Relief in the hand and the chest is physical, but a change in self-talk touches the very pattern I had read as the two faces.

My Observations as Therapist

First, and the biggest.

The client said that when he wants to speak, his throat closes. And his presenting symptom is that when he wants to write, his hand stiffens. The beginning was there too, in the PSC Mains examination, where he had to prove himself by writing.

There are two doors of expression. Both of them shut. The hand while writing, the throat while speaking.

I think this is the central thread of this case. It is only my inference, not proof, but the connection between the two does not seem worth ignoring.

Second.

In every scene Abhinav appeared alone. Alone at the seashore, alone in the field, laborer’s were present but not one of them so much as looked at him. And in every scene he said that everything is fine, I am happy, I feel peaceful.

This is the same face he has kept showing to the outside world. That same face was present in the trance as well. The CM did not let the door of the SCM open.

Third.

One thing was common to both scenes, a water body. Once a sea, once a pond. Perhaps this is that frozen depression of his which the conscious mind does not want to show, but which the subconscious was showing in the form of a metaphor.

This interpretation seems possible to me, but I do not hold it as firmly as the first observation.

Fourth, a moment I let slip.

In the whole session, Abhinav changed his tone only once. When I asked whether he felt lighter after releasing the box, he said, I cannot say.

Everywhere else he was saying that it felt good. Only there, not.

I comforted him and moved on. Now I feel that was the place to stop. That I cannot say was not an end, it was an entrance.

Fifth, a reconsideration of my own suggestion.

Towards the end of the session I had first thought of saying, now you are completely free of that burden, completely free. At that moment it felt natural.

But on thinking it over, it seemed that for a man whose entire problem is that he walks about wearing an everything-is-wonderful face before everyone, saying this would have been an invitation to yet another performance. A performance of being well, this time. So at the end I left it as an open possibility instead.

Overall assessment.

In the direction of Prati-Prasava, that is towards the originating experience of the symptom, this session did not reach. Even the first stage of IDT was not completed, because the client could not connect with the experience at all. Res. was therefore not to be expected.

What I did get is this. Where the resistance sits, and in what form it sits, is now clearly visible.

My Remaining Questions

Would this be called a meaningful session?

Despite pain this deep, why could the client not reach its source? Was this resistance, or a limitation of my technique?

The path of healing that I chose, was it the right one? At that turn, would it have been better to try fractionation or signaling through IMR?

Would we call this a past life regression session?

Therapist’s Note

A warrior armed with the sword of arguments and the shield of fear, standing alone with two faces and one body. Wounded by his own weapons, prisoner of his own reasoning. A quiver of questions on his back, and on his tongue the slogan, all is well.

Will that wall of deadlock break? Will he be able to meet that enemy who is piercing him through with invisible spears?

It did not happen this time. But I now know where that wall stands.

Focal dystonia : a neurological disorder in which involuntary contractions, spasms or stiffness begin to occur in only one specific part of the body or group of muscles.

Congratulations @satishsinghthakur Ji on your second report  Some sessions may not end as we expected. But still there will be a lot to learn from that for our future journey as a therapist. Waiting for the suggestions from venu Ji and experienced Amarantos members.

Wow, great Satish ji! The way you handled and shared is so beautiful and in detail. I can easily connect with it while reading. I remember Venu Sir saying that healing can happen naturally and doesn’t always have to be through past-life experiences. The way you spent time in pre-talk, history taking, rapport building, and suggestions to the client through the session is what a therapist should do. And your observation of the details was also excellent. Great work and great learning for all of us. My best wishes, Satish ji! :folded_hands::lotus:

Precise and accurate observation. :100: true! I guess this is the निचोड़ of this case. Person is too closed in his own mind and resistance showed in the sessions. It’s hard for him to open up. He needs pre session therapy and it’s going to be hard from him to actually jump straight to PLR. @satishsinghthakur Satish, you did wonderful! Full marks for honestly :folded_hands:t2: you didn’t glorify the write up and presented as it is. You tried all the tools and techniques in each and every possible way. That cliff reframing by the pond was very nicely improvised. Your client is lucky to get the opportunity to had a session with you. You were empathetic, kind, caring and supportive. You are very patient. You let the session flow but tried to anchor it whenever you found the right opportunity/key moments. Great going :+1: you have been refining your skills remarkably :folded_hands:t2::cherry_blossom::cherry_blossom: way to go

Thank you so much, Vignesh ji, for your thoughtful words and encouragement. :folded_hands:

I’m especially glad you mentioned Venu Sir’s teaching that healing can happen naturally and does not always have to come through a past-life experience. That understanding has been an important part of my learning, and this case reinforced it for me in a very practical way.

Your observation about the pre-talk, history taking, rapport building and the way the session was handled means a lot to me. I’m happy that these aspects came through in the case presentation.

Every case teaches us something new, and I’m grateful to Venu Sir and the entire learning community for that. Thank you once again for your generous wishes. :folded_hands::cherry_blossom:

Thank you so much, Harmeet ji, for such a thoughtful and encouraging response. :folded_hands:

Your observation about the client being too closed within his own mind and the need for pre-session work really adds another dimension to the case. I completely agree that sometimes the real work begins even before PLR.

I’m especially grateful that you noticed the honesty in the case presentation. For me, sharing the resistance, limitations, and what didn’t work is just as important as sharing the breakthroughs. Every such session teaches us something and helps us refine our understanding and skills.

Thank you once again for your generous words and encouragement. Truly appreciate it. :folded_hands::cherry_blossom:
